The Hidden Pouch can be found when killing enemies while using a Tymora's Lucky Enchantment from the Tymora's Gift event.

Stacks to 100.

The pack's contents can include:

[Peridot]
[White Pearl]
Uncommon Ring or Necklace
Healing or Buff Potions
Treasure
A Random amount of Gold

Getting enough of Tymora's Lucky Enchantments to increase the drop percentage to over 5% means that about every 20th enemy you kill will drop a Hidden Pouch, which includes enemies killed in pvp.
